How to Request Service
Please have the following information available to complete your request. For a list of forms, see Additional Resources below.
Forms including sensitive data must be mailed to SSC HR. University policies, contractual obligations, and federal and state laws and regulations require appropriate protection of PPI, including transmission via any type of media.

- UMID number

What needs to be included on the 'personnel file only' documents I submit?
UMID must be typed on the document (upper right hand corner).
Why does the UMID need to be typed on all HR documents?
The UMID is used to electronically catalog the documents. Missing UMIDs and/or illegible handwritten UMIDs can result in delayed processing or creating an incorrect transaction.
Can I include notes on a form?
Documents are recorded electronically so please do not include or attached notes.
Can I use any version of a form?
You must use the most recent version to avoid delayed processing.
Why was my transaction denied?
Transactions may not be able to be processed for many reasons. You should have received an email notification or a workflow denial with details about the reason for the inability to process. You can contact firstname.lastname@example.org to get further clarification.
