Mortgage Companies – This section applies to you if you are a mortgage company, a lender, or a third party acting on behalf of a lender for mortgage purposes (i.e., Veri-tax, Equifax, Quicken Loans, etc.)

Please use this eForm to request employment verifications. You will be asked to provide the following information: employee name, DOB, SSN, or UMID. You will need to include a copy of the employee’s authorization and the form for completion. Click here for a list of verifications we cannot provide.

Note: The SSC completes a digital version of Fannie Mae Form 1005. To decrease processing time, we do not include boxes 1-6 in our response. Please click here for an example of the SSC’s verification of employment form for mortgages.

Due to company policy, we cannot answer the following questions:

*Box 11 – Probability of continued employment

*Box 14 – If overtime or bonus is applicable, is its continuance likely?

*Box 16 – Date of applicant’s next pay increase

*Box 17 – Projected amount of next pay increase

*Box 24 – Reason for Leaving

Third Parties – This section applies to you if you are a background screening company or a company looking for general employment verification not related to mortgage purposes (e.g., Sterling Talent Solutions, HireRight, Certiphi, Business Information Group, etc.)

Please use this eForm to request employment verifications. You will be asked to provide the following information: 

• Employee name
• Date of Birth
• Social Security Number or U-M ID number
• A copy of the employee’s authorization and the form for completion.
