Any cardholder who requires building access to Michigan Medicine facilities must obtain a Michigan Medicine MCard (Available at the Key & ID Office and NCRC Station) regardless of whether they have access at another campus.

Ann Arbor Campus departments must complete the Ann Arbor Campus MCard Request and Change eForm for a new Ann Arbor MCard for the following people who do not  require Michigan Medicine or the North Campus Research Complex building access:

  • faculty – if MCard required before the official start date
  • staff – if MCard required before the official start date
  • U-M temporary staff
  • retirees
  • contractors
  • contracted temps (Manpower, Kelly Services, etc.)
  • vendors
  • visitors

This form is generally NOT required for:

  • students, alumni, visiting scholars (a.k.a. Academic Affiliates) or volunteers (considered visitors)
  • replacement MCards (lost, stolen, expired, or personal name changes)

It is required for the replacement of expired cards for contractors, contracted temps, temporary employees, vendors and visitors.

After the individual’s department completes the MCard Request eForm, the indvidual can pick up their MCard at any Ann Arbor campus MCard issuing station with government photo ID (i.e., driver’s license, state ID, military ID or passport).

Online photo submission is available for new Ann Arbor students.

New students receive their MCard (no cost) during orientation.  Students must be matriculated and term activated. New students will not need a form to obtain their MCards (except for Medical, Nursing, PIBS and Pharmacy students requiring Michigan Medicine access).

Students who have a previous MCard will be required to turn it in to obtain their new student MCard, otherwise a $20 replacement fee may apply. Students who still have a valid, unexpired student MCard can continue to use that card until it expires. Student MCards are valid for five years. Undergraduate and graduate MCards both indicate STUDENT.

MCard Issuing Stations:

New employees will generally obtain their first MCard (no cost) after their official start date. Faculty and staff may also obtain their MCards at an MCard Issuing Station with a completed online MCard Request and Change form by their department if their employee affiliation is not in the MCard system (prior to their start date).

People who have had a previous MCard will be required to turn it in to obtain the employee MCard, otherwise a $20 replacement fee may apply. Employee MCards are valid up to five years. The university department or unit name will print under cardholder’s name but not their title. Employees with multiple appointments will choose the most appropriate single department/unit name to be printed on their MCards. There is not a means of printing multiple department/unit names on MCards.

MCard Issuing Stations:

U-M Temporary Staff

U-M units are required to complete an online MCard Request and Change eForm for individuals hired as temporary employees from either U-M Temporary Staffing Services or a U-M department for their first free Temporary MCard. Those that have had an MCard previously will be required to turn it in to obtain the new MCard, otherwise a $20 replacement fee may apply. Temporary MCards are valid for up to one year.

Campus Contracted Temp, Contractor, Vendor, & Visitor MCards

Ann Arbor U-M units are required to complete an MCard Request and Change eForm for these individuals.  They will need to pay $4.00 for the MCard unless the unit sponsors it with a valid shortcode. MCards are valid up to one year.

Those that have had an MCard previously will be required to turn it in to obtain the new Mcard, otherwise a $20 replacement fee may apply. To replace an expired MCard, an MCard Issuing Station will require new MCard Request and Change forms, $4.00 and the expired MCard.

Alumni can obtain an Alumni MCard at any MCard Issuing Station (except for the Michigan Medicine Key & ID Office). MCard Issuing Station staff will verify Alumnus status and require a payment of $20. Alumni will not need to complete any forms.

Alumni MCards are valid for five years. Alumni MCards do not have any implied benefits or access. Expired Alumni MCards are not required to be turned in to an MCard Issuing Station for a new Alumni MCard, although each new Alumni MCard requires a payment of $20.

MCard Issuing Stations:

University retirees, including professors emeriti, from any U-M campus can obtain their first Retiree MCard for free from the Shared Services Center (SSC) MCard Issuing Station. A completed MCard Request and Change eForm by unit management or Human Resource retirement letter is required for the production of the Retiree MCard before retirement. No documentation should be required if seeking a Retiree MCard after retiring.  Retirees will be required to turn in employee MCards to their department on their last day or when the Retiree MCard is issued.

Retiree MCards are valid for five years. After five years, retirees can exchange an expired card for a new card. Expired MCards must be turned in, otherwise a $20 replacement fee will apply.  If a replacement fee is required, retirees must make payment via credit card using the following link and evidence of payment must be provided (printed receipt or photo) with the request for a new MCard.  IMPORTANT: When submitting a credit card payment, place the word “MCard” in the “invoice” field.

Expired Retiree MCards can be replaced by one of two methods:

  1. MCard replacement via U.S. Postal Service
  • Expired MCard is sent by mail to: U-M Shared Services Center, 1000 Victors Way, Ann Arbor, MI 48108, along with a letter indicating your request for a new MCard, mailing address to send the new MCard and a recent photo of yourself.  Please include a preferred phone number or email address in the event the SSC needs to contact you.
  • SSC will print a new MCard and return it through the U.S. Postal Service First Class mail.

     2.  In person MCard replacement through the SSC

  • Please initially send an email to [email protected] with
  • email subject line: Replacement MCard Request.
  • Attach a recent photo (headshot only), preferably in jpg format, to be printed on your new MCard.
  • Include your eight digit UMID number and uniqname (if you have an active U-M email account).
  • Please include a preferred phone number or email address in the event the SSC needs to contact you.
