Michigan Medicine MCard Information and Issuing Stations
All persons affiliated with Michigan Medicine are required to wear a current Michigan Medicine MCard. Michigan Medicine MCards are issued at the Key & ID Office in the Med Inn Building and the NCRC MCard Issuing Station in Building 18 of NCRC. If a person requires access to the Michigan Medicine and Ann Arbor, Dearborn, or Flint campuses, the cardholder must still obtain their MCard at the Key & ID Office or NCRC MCard Issuing Station. Michigan Medicine MCards provide access to Michigan Medicine and campus buildings (even with smart card building readers). However, campus MCards do not provide access to Michigan Medicine buildings.
A completed & signed Michigan Medicine MCard Request and Change form by Michigan Medicine management is required for all new and replacement MCards with the exception of damaged MCards and expired MCards for full-time employees. The Key & ID Office and the NCRC MCard Issuing Station do not accept MCard fee payments. A receipt of payment will be provided which will need to be shown at the MCard issuing station as proof of payment.

No Campus MCards are issued at Michigan Medicine MCard Issuing Station locations
Please do not send groups of ten or more individuals to an MCard Issuing Station who need MCards. Contact [email protected] for group MCard production options.
Any cardholder who requires building access to Michigan Medicine facilities must obtain a Michigan Medicine MCard (Available at the Key & ID Office and NCRC MCard Issuing Station) regardless of whether they have access at another campus.

Requesting New and Replacement MCards
A completed & signed Michigan Medicine MCard Request and Change form by Michigan Medicine management is required for all new and replacement MCards with the exception of damaged MCards and expired MCards for full-time employees.
Expired MCards
Full-time and part-time employees with expiring/expired MCards need to exchange their MCards for new ones. If full/part-time employees still have an active profile in the MCard system and there are no changes, they will NOT need an MCard eForm request submitted.
However, if there are changes, to department, job title (credentials), access changes or the MCard has been expired longer than 30 days, then an authorized signer will need to submit a request through the Michigan Medicine Help Center.
All other MCard holders with expiring/expired or other changes will need to bring:
- A new photo is required if the previous MCard photo is over 5 years old.
- Have an eForm request submitted on their behalf by an authorized signer through the Michigan Medicine Help Center
- Their current MCard to a Michigan Medicine MCard Issuing Station
These cardholders are required to bring a completed and signed Michigan Medicine MCard Request and Change form by Michigan Medicine management to the Key & ID Office or NCRC MCard Issuing Station along with a paid receipt from the Hospital Cashier’s Office for $6.00 for an MCard. Some Michigan Medicine units sponsor the $6.00 costs with internal billings.
People who have a previous MCard will be required to turn it in to obtain the new MCard, otherwise a replacement fee may apply. MCards are valid up to one year. The Michigan Medicine department or unit name will print under cardholder’s name.

- Have an eForm request submitted on their behalf by an authorized signer through the Michigan Medicine Help Center
